An excellent Long and Short Term Care Facility (LTC/STC) is looking for a Physical Therapist / PT to join their team.
Requirements needed: New Grads or Experienced Therapists Needed .
The Physical Therapist / PT is responsible such services as: patient assessment, formulate patient care plan, execute patient care plan, keep records, and family education.
Requirements include:
* Graduated from an accredited school.
* Current state license as a Registered Physical Therapist / PT or in the process of obtaining a state licensure as an Physical Therapist / PT or currently in the application process of applying for a Registered Physical Therapist / PT licensure.
* In accordance with physician orders: Provide skilled physical therapy services / interventions / modalities / ROM / family education.
* Collaborates with a multi-disciplinary team to evaluate and re-evaluate goals for each physical therapy patient.
* Evaluate and assess each physical therapy patient needs and create a physical therapist treatment plan to meet the desired goals to achieve maximum independence.
